Underground Locate Request Form

This Underground Locate Request is for University of Saskatchewan owned infrastructure only. The Service Requestor must always also contact Sask 1st Call and any other companies who may have underground facilities in the work area www.sask1stcall.com as they cannot be located by the university.

Please complete the Underground Locate Request form in full and submit, including map using Internet Explorer or Google Chrome. If you have any questions, please contact Facilities Support Services at 306-966-4496.

Completed forms go directly to Facilities Support Services, where they are reviewed and forwarded to the required service area(s). Requester should allow up to five business days for the locate in “Locate to be complete by date".
